Women’s Healthcare
Primary Healthcare
Well adult and well child visits, problems visits, and general healthcare by a Family Nurse Practitioner
Psychiatric Care
Diagnosis and management of mental healthcare needs including substance use disorder and medication assisted treatment
Prenatal and Postpartum Care
Patient centered prenatal care and care post birth. We will arrange for an inpatient care provider during hospital stays.

WHO WE SERVE
WOMEN OF CHILDBEARING AGE WHO ARE:
- At risk for substance use disorder (SUD)
- Currently experiencing SUD
- Receiving medication-assisted treatment (MAT)
IMMEDIATE FAMILY MEMBERS
of these women may also be eligible for services.
There are many risk factors for substance use disorder. Some of these may include: a family member with a substance use disorder, low income, divorced or separated, unemployment, history of physical, mental, or sexual abuse, adolescent age, housing insecure, and partner or parental stress/discord. Women with a history of chronic pain, anxiety, depression, or mental illness are particularly vulnerable and are candidates for these services. Care is not based upon insurance coverage. You may enter care with HHH regardless of your medical insurance situation.
Case Management
Our case management services offer personalized, client-centered support to help individuals navigate mental health related challenges. We assess needs, develop tailored care plans, connect clients to vital resources, coordinate with providers, and monitor progress—empowering individuals to overcome barriers, reduce crisis, and achieve greater independence and well-being.
