We are committed to providing high quality care to everyone in our community, regardless of their insurance status or ability to pay. We offer flexible payment options and financial assistance to ensure our services remain within your reach, regardless of your current financial situation.

Sliding Fee Scale

For those who are self-paying or need assistance with their bill, we offer a SLIDING FEE SCALE. This program allows us to reduce your costs based on your household size and income level. Income is defined as your gross (before taxes) monthly income based on individual or household (work, social security and unemployment, etc). Household size is composed of adults and children under the age of 18 residing in the home.

RE-VERIFICATION POLICY: To ensure you continue receiving the correct discounted rate, those enrolled in the Sliding Fee Scale program are required to re-verify their income every six months. You may also re-apply sooner if there is a significant change in your family size or financial situation.

INSURANCE ACCEPTED: Medicare, Medicaid, and most major commercial insurance plans.

In compliance with the No Surprises Act, this practice provides a Good Faith Estimate of expected charges to all uninsured or self-pay patients prior to service, and ensures all services billed under this sliding fee scale are free from surprise billing.
