Helping children find success at home, school, and the community

Pathways to Success is a program for Medicaid enrolled children under the age of 21 in Illinois who have complex behavioral health needs and could benefit from additional support.

The program provides access to an evidence-informed model of intensive care coordination and additional home and community-based services. Pathways to Success is here to help your family with areas of conflict or crisis while also keeping your child and family together at home.

We Help

Improve your family’s relationship
Create a more stable living situation
Increase your family’s involvement in behavioral health services
Improve school attendance/performance
Reduce contact with law enforcement and child welfare

Who Is Eligible?

Be under 21 years old
Be an Illinois resident
Be enrolled in Medicaid
Have a Severe Emotional Disturbance (SED) or Severe and Persistent Mental Illness (SPMI)
Be placed into Tier 1 or 2 of the Illinois Behavioral Health Decision Support Model

When we begin working with your family, we will complete a Strength, Needs, and Cultural Discovery assessment. By telling us your family’s story, we learn how to best help you.

We help you create a Crisis Prevention and Safety Plan (CPSP). Your family will learn what a crisis is, how to reduce the intensity of a crisis, and how to overcome a crisis.

Together, we also help you create a Child and Family Team. Members of the team could be family, friends, co-workers, neighbors, mentors, coaches, and more. The team meets regularly to create a unique care plan for your child.
